1. Read the Gifts and Entertainment (G&E) Policy (linked below)

2. Click the 'Continue' button at the bottom of the page to submit the Gifts and Entertainment online form.

  • Affiliated Individuals will be required to log-in to SportsEngine (or create an account) in order to advance in this online form.
  • Any gifts and/or entertainment should be included in the form (this form covers both--approval to be given or received).

3. Save the confirmation email received after submitting this disclosure so periodic statements or updates can be submitted as soon as possible when any new gifts and/or entertainment potential arises.

Each form will be reviewed and addressed as outlined in the G&E Policy.

4. Submit updates as needed - Submit a new form as soon as possible when any new gifts and/or entertainment potential arises.

5. Request and/or Refuse as required
Know the G&E Policy. The only permitted gifts or entertainment are those outlined in the G&E policy, and those gifts or entertainment must be properly approved and/or disclosed.

The NWBA has adopted this Gifts and Entertainment Policy to provide instruction to Affiliated Individuals regarding how to handle offers of gifts or other forms of entertainment from individuals or companies that do business with, or are interested in doing business with the NWBA. Additionally, it provides instruction on the NWBA extending gifts to third parties.


Affiliated Individuals

This Gifts and Entertainment Policy is applicable to all NWBA employees, board members, officers, committee members, task force members, hearing panel members, and volunteers with substantial decision-making authority, as well as their spouses and immediate family members. Where reasonable, NWBA contractors may be required to comply with this policy in their contracts. (“Affiliated Individuals”).
